I believe that even though most of the cites are lowercase the canonical form of this should be uppercase. Sometimes people lowercase proper nouns to show disrespect, and we wouldnt make an entry for each of them, e.g.

greek {{lb|en|derogatory}} Greek.

This could be argued to be a different situation because it's a set phrase, but I still think it should be uppercase as is its synonym Koran thumping and its coordinate term (and probably origin) Bible thumping.
